Incident Investigation Survey

Incidents that merit meaningful investigative attention are few and far between, thankfully, but the infrequency of them is also part of the problem – for every serious incident that creates audible noise, there are 10’s, even 100’s of other incidents that act as weak signals to those few events.

  • Knowing which ones to pay attention to is the first challenge.

  • The second challenge is to extract maximum learning from unplanned events in a way that enables leaders to accurately target their efforts at improving control and reducing exposure to risk.

  • The final challenge is in the design of incident information management systems that produce accurate and close to real-time business intelligence on how well exposure is being managed.
