Incident Investigation Survey
Incidents that merit meaningful investigative attention are few and far between, thankfully, but the infrequency of them is also part of the problem – for every serious incident that creates audible noise, there are 10’s, even 100’s of other incidents that act as weak signals to those few events.
Knowing which ones to pay attention to is the first challenge.
The second challenge is to extract maximum learning from unplanned events in a way that enables leaders to accurately target their efforts at improving control and reducing exposure to risk.
The final challenge is in the design of incident information management systems that produce accurate and close to real-time business intelligence on how well exposure is being managed.
